The Serious Reportable Event (SRE) compliance clock

Once an SRE is identified, four deadlines corresponding to licence conditions start running.

On identification

Preliminary assessment

Immediately assess whether the harm could spread to more people, and put interim containment measures in place while the SRE Quality Assurance Committee (QAC) investigates.

2 working days

Notify MOH

Notify MOH's Clinical Quality, Performance and Value Division via the National Quality Assurance System (NQAS), including the date, location, and a summary of the event.

60 calendar days

Submit the RCA report

The SRE Quality Assurance Committee completes its root cause analysis and submits findings and recommendations to MOH. Or 30 calendar days to resubmit if MOH finds the report inadequate.

Half-yearly, until closed

Report on implementation

Submit progress reports on outstanding recommendations every six months until every corrective action has been implemented.

Enforcement

Under section 20 of the Healthcare Services Act, a contravention can mean licence suspension, licence revocation, or a financial penalty of up to $10,000 per contravention.

What is a Serious Reportable Event (SRE)?

A Serious Reportable Event (SRE) is one of 31 specific categories of serious adverse events and patient safety incidents defined in MOH Circular 12/2023. It includes surgery on the wrong site or patient, retained foreign objects, medication errors causing serious or permanent harm, and patient abscondment or suicide resulting in death or serious injury. Any SRE must be reviewed by a dedicated SRE Quality Assurance Committee (SRE QAC) and notified to MOH within 2 working days of identification.

Is incident reporting mandatory for nursing homes and care organisations in Singapore?

Yes. Since 6 March 2023, MOH Circular 12/2023 has made structured review of Serious Reportable Events a binding licence condition for prescribed licensees under the Healthcare Services Act. Separately, AIC's June 2025 Tripartite Implementation Guide adds standardised protocols for abuse and harassment incidents specifically (organisations are strongly encouraged, not yet legally compelled, to adopt it), and the Health Information Act imposes its own, unrelated 2-hour/14-day reporting clock for data breaches and cybersecurity incidents.

What happens if a facility misses the MOH SRE notification or reporting deadline?

MOH Circular 12/2023 gives the Director of Medical Services the power to take regulatory action under section 20 of the Healthcare Services Act for any contravention, including suspending or revoking the facility's licence, or imposing a financial penalty of up to $10,000 per contravention. The circular sets hard deadlines: 2 working days to notify MOH after an SRE is identified, 60 calendar days for the SRE Quality Assurance Committee's root cause analysis report, and half-yearly progress reports until every recommendation is implemented.
